About The Position

The RN Supervisor assists in implementing and evaluating health and personal care services for RPH elders in accordance with current federal, state, and local standards and regulations and RPH policies and procedures to ensure that the physical, emotional, and social needs of the elders are met and maintained on an individual basis. RPH serves elders with memory impairment and is licensed and certified by the NYS DOH to provide ALR, SNALR, EALR levels of care. The RN Supervisor serves as a member of the interdisciplinary team and is (or will be) specifically trained in the supportive care of people with memory impairment, dementia, and other age-related diagnoses to effectively implement elders’ individualized wellness care plans.

Requirements

  • NYS license as RN in good standing. Must adhere to the Nurse’s Code of Ethical Practice and Conduct and practice under the scope authorized by the Nurse License.
  • Have 1-2 years’ experience in long-term care, assisted living community or ALR homecare setting working with older adults. One year in a management or supervisory capacity.
  • Able to assess psychosocial needs, work with colleagues to determine care needs and communicate factually and reliably to a variety of professionals and family members.
  • Able to work independently and collaboratively as part of a team. Possess ability to make mature, independent decisions in a timely manner.
  • Be proactive, take initiative, resolve problems, follow through and manage multiple priorities and work well under pressure.
  • Possess interpersonal and communication skills to communicate tactfully with personnel, residents, family members, visitors, government regulators and the public.
  • Be a quick learner, empathetic, compassionate, and savvy with ability to preserve the dignity and self-image of residents in their presence and a desire to achieve goals to make a difference in the lives of others.
  • Ability to work flexible hours to meet deadlines, including evenings and weekends.
  • Ability to maintain confidential information.
  • Valid driver’s license

Nice To Haves

  • 1 year of elder care or direct care experience is preferred but not required.

Responsibilities

  • Deliver health care and personal care services to elders with memory impairment and act in a leadership position to guide and coach Care staff in service delivery.
  • Supervise, provide assistance and oversight with medication management and administer routine and prn medications including diabetic management, insulin injections and blood glucose tests in compliance with nursing standards and NYS regulations.
  • Assist care coordinator in ensuring that medical appointments, specialty consults and discharge summaries are followed up, communicated and documented in the medical record.
  • Set-up medical record upon new admission; medication list, MAR, admission note, service delivery record, etc.
  • Review medication records on a monthly and as needed basis to ensure that all new orders have been carried over and transcribed accurately.
  • Participate in resident/family meetings to help create and revise wellness plans. Participate in meetings to evaluate elders’ needs and progress. Communicate with Shahbaz/Care Partners and ensure that they are knowledgeable of elders’ wellness plans and using these plans to direct daily care.
  • Document medical and personal care services provided.
  • Communicate changes in resident condition/orders through 24-hour report to Case manager.
  • Facilitate huddles with nursing and direct caregivers on a daily basis to evaluate and communicate elders’ needs and progress.
  • Communicate service delivery and elder response to care to the RN Case Manager and MD.
  • Communicate with colleagues to ensure elder care is seamless and effective.
  • Make daily rounds on all floors to ensure household roles are completed. Assist as needed.
  • Document resident incidents, assist in investigations analyze for root cause and implement interventions to prevent reoccurrence.
  • Be knowledgeable of DOH regulations and ensure compliance.
  • Create and maintain a warm atmosphere of positivity and calmness. Ensure residents are treated with dignity and respect and that resident rights are maintained.
  • Coach care partners to engage with elders in elder-directed daily routines and assist with both scheduled and spontaneous engagement activities. Encourage elder participation in social/emotional and physical activities as appropriate.
  • Dine with the elders as often as possible.
  • Oversee compliance with written policies and procedures in accordance with RPH, federal and state regulations.
  • Ensure that work areas are kept in a clean, safe, and sanitary state. Ensure elder’s rooms are clean/neat and in compliance with regulatory and RPH standards.
  • Take a lead role in welcoming and orienting/training new staff. Provide coaching and guide care staff as needed. Provide praise and recognition for a job well done. Foster positive interpersonal relationships.
  • Investigate complaints and grievances made by elders or families and communicate to leadership.
  • Provide written and/or verbal reports and updates to the RN Case Manager, Social Worker or Administrator as required.
  • Participate in survey process (inspections) made by authorized government agencies.
  • Be prepared to handle emergencies as they arise and take on-call responsibility as assigned.
