About The Position

WashU is seeking a highly skilled and adaptable Staff Registered Nurse (RN) to support the Division of General Medicine and Geriatrics, primarily supporting Geriatric Primary Care and the Geriatric Assessment Clinic. This hybrid role offers both in-clinic and hybrid settings and focuses on triage nursing, requiring the ability to manage a high call volume, coordinate care efficiently, and multitask in a fast-paced environment while ensuring exceptional patient care. In this role you will work closely with physicians, advanced practice partners, and other healthcare professionals to optimize patient outcomes, offer patient education, symptom management, and care coordination. This position also participates in the assessment and management of geriatric syndromes, including cognitive decline, falls, and polypharmacy concerns.

Requirements

  • Associate degree - Nursing
  • Basic Life Support certification (Online BLS certifications, those without a skills assessment component, are not sufficient to meet the BLS requirements).
  • Current RN licensure in the state of Missouri and/or Illinois, depending on location, in accordance with Compact Act.
  • Basic Life Support - American Heart Association, Basic Life Support - American Red Cross, Registered Nurse - Illinois Department of Financial and Professional Regulation, Registered Nurse - Missouri Division of Professional Registration

Responsibilities

  • Provide comprehensive nursing care and support to geriatric patients in accordance with established clinical guidelines, policies, and standards of practice.
  • Conduct telephone triage assessments via incoming and outgoing calls, evaluate patient symptoms and concern, provide appropriate nursing advice, and escalate urgent issues to providers when necessary.
  • Manage incoming and outgoing patient phone communications, phone triage, including appointment scheduling, follow-up coordination, and communication of test results and provider recommendations.
  • Review, prioritize, and respond to patient portal (MyChart) messages in a timely and professional manner, including triaging clinical concerns, facilitating provider review, and coordinating follow-up care.
  • Prepare, pend, and process medication refills, laboratory orders, diagnostic testing orders, referrals, and other provider-directed orders following established protocols.
  • Review patient medical records and clinical documentation to ensure appropriate medication refill requests and continuity of care.
  • Complete and process a variety of patient-related forms and documentation.
  • Facilitate electronic document completion and signature workflows through DocuSign and other approved electronic platforms.
  • Request, obtain, review, and maintain external medical records to support ongoing patient care, treatment planning, and provider review.
  • Coordinate communication among patients, caregivers, providers, specialists, pharmacies, and community resources to ensure continuity of care.
  • Educate patients and family members regarding treatment plans, medications, disease management, preventive care, and available healthcare resources.
  • Maintain accurate, complete, and timely documentation within the electronic health record (EHR) system.
  • Assist with care coordination, chronic disease management, and follow-up for geriatric patients with complex medical needs.
  • Ensure compliance with HIPAA regulations, organizational policies, and applicable state and federal healthcare requirements.
  • Participate in quality improvement initiatives and support a patient-centered approach to care delivery.
  • Other duties as assigned.
