In accordance with the Vision, Mission and Values of Fraser Health, and the British Columbia College of Nurses and Midwives (BCCNM) standards of practice, the RN/RPN Liaison works collaboratively and in partnership with the interprofessional care team in the provision of person-centered, trauma-informed practice and a recovery centered clinical system. Initiates clinical psycho-social -bio-spiritual assessments of, consultation and crisis intervention to patients in an inpatient unit of the hospital, or in the Emergency Department (ED) for substance use concerns, with regard to mental health and psychiatric disorders. Assists with patient admission and transfer to ensure services and bed destination is achieved in a timely manner. Within the context of a patient-centered care, screens and assesses patients to develop care plans. Collaborates and assists in adapting care plans to address substance use care needs. Acts as a resource to the clinical areas of the hospital site, the Mental Health and Substance Use Services (MH&SUS) programme and regional committees as required. Provides input into and assists with ensuring effective patient flow for assessed and admitted patients including those awaiting transfer for further assessment.
