The RN works on 4IC, a 32-bed progressive care unit (PCU). Patients commonly include post-catheterization patients requiring sheath and TR Band management, patients with medical conditions requiring telemetry monitoring, and overflow patients from 3E who may require chest-tube care. TAVR experience is preferred. The unit's nurse-to-patient ratio varies with acuity and is approximately 1:4–6. Nurses titrate all drips except pressors, paralytics, and TPA. Centralized telemetry monitoring is available, and RNs read rhythm strips once per shift and when a rhythm change occurs. CPAP, BiPAP, and high-flow oxygen are available on the floor; RNs place patients on and off these therapies, while Respiratory Therapy sets the rates and is available in-house to assist. The unit has a charge nurse on each shift, who most often also takes an assignment, and care technicians at approximately a 1:15 ratio. Scheduling is self-scheduled but may be changed to maintain a balanced schedule. Weekend requirements are six weekend shifts in a six-week period; holiday requirements are two summer and three winter holidays. There is no call or standby requirement. RNs may be floated to the floors. The scrub color is galaxy blue. Unit-specific orientation is one to two shifts.
