About The Position

Generations Hospice is seeking a passionate, dedicated Hospice RN to join our team! Our hospice RNs provide expert, patient-centered care at a critical time in people’s lives. This role offers lower patient-to-nurse ratios for meaningful, unrushed hospice care, strong interdisciplinary team support, and leadership that understands hospice nursing. A dedicated internal after-hours triage team supports nurses outside of regular business hours. The position utilizes Homecare Homebase (HCHB) EMR for streamlined documentation. The office is located at 1028 S. Main Street Suite C Fond du Lac, WI 54935, with a coverage area including South Fond du Lac areas - Columbus/Fall River/Beaverdam/Mayville. The schedule is Monday-Friday 8AM-5PM. This role offers greater work/life balance with flexible scheduling options, less time on your feet compared to other settings, and the ability to work independently while also having team support, along with job stability and regular advancement opportunities with a growing company.

Requirements

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order

Nice To Haves

• 3+ years preferred nursing experience
• Experience in hospice or a similar setting preferred

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patient's wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient’s family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch
