About The Position

Allay Hospice is seeking a passionate, dedicated Hospice RN to join our team! Our hospice RNs provide expert, patient-centered care at a critical time in people’s lives. The role involves lower patient-to-nurse ratios for meaningful, unrushed hospice care, strong interdisciplinary team support with collaborative care with Social Work, Chaplaincy, Aides, and providers, and leadership that understands hospice nursing. A dedicated internal after-hours triage team supports nurses outside of regular business hours. Homecare Homebase (HCHB) EMR is used for streamlined documentation. The office is located at 325 N Corporate Dr Suite 260, Brookfield, WI 53045, with coverage required throughout the territory, primarily Waukesha and Milwaukee Counties. The schedule is Full Time Monday-Friday 8AM-5PM.

Requirements

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order

Nice To Haves

• 3+ years preferred nursing experience
• Experience in hospice or a similar setting preferred

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patient's wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient’s family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch
