Requirements

  • Master's in Nursing (MSN) or Bachelor’s in Nursing (BSN) and must attain MSN within 18 months of hire.
  • Minimum of 2 years of experience on a Stroke Designated unit or working with Stroke patients.
  • Requires NIH Stroke Scale Certification (or must obtain by end of orientation)
  • Requires AHA BLS - Healthcare Provider
  • Requires Registered Nurse - NJ
  • Requires ACLS (or must obtain within 6 months of hire date) if assigned to: Critical Care/Intermediate/Telemetry, Emergency Rooms, Pediatrics/Pediatrics Emergency Room, Labor & Delivery, Surgical Services (not to include Perioperative), Interventional Procedures, Observation.
  • Requires NRP (or must obtain within 6 months of hire date) if assigned to: Maternity Services
  • Requires PALS (or must obtain within 6 months of hire date) if assigned to: Emergency Rooms, Infant Follow-Up, Surgical Services (only PACU & Same Day Surgery), Pediatrics/Pediatric ED.
  • Intermediate computer skills in Microsoft Office.
  • Extensive familiarity with use of internet as a clinical and administrative resource.
  • Ability to form and maintain interpersonal professional working relationships

Nice To Haves

  • SCRN, ASC-BC preferred.
  • 8 hours of Stroke related Continuing Nursing Education (CNE) contact hours annually

Responsibilities

  • Manages and coordinates disease specific care from moment patient arrives to hospital unit discharge and/or along the entire continuum of care.
  • Responds to any pertinent disease specific alert during work day and collaborates with healthcare team to promote timely access to care according to national standards and guidelines that can significantly improve outcomes for patients.
  • Manages implementation of respective care protocols as defined by Capital Health (CH).
  • Coordinates revisions, changes and addition of all care related supplies and equipments throughout Capital health.
  • Monitors patient while hospitalized at Capital Health until successful discharge of patient, including but not limited to providing daily disease specific census data to appropriate patient care management team.
  • Facilitates development and implementation of education for patients and families.
  • Assesses, manages, implements, and evaluates interventions set forth by state and national regulatory bodies, CMS and Joint Commission disease specific program certification.
  • Collaborates with Nurse Managers, Clinical Specialist and Clinical Educators to mentor and educate staff to promote successful implementation of appropriate care standards.
  • Maintains constant readiness for system-wide Regulatory Agency surveys and inspections as measured by self-assessment against standards.
  • Collects data related to Regulatory Agencies for appropriate disease process using approved data collection tool.
  • Reviews medical records of all cases for achievement of quality and outcome criteria.
  • Engages staff in active ongoing participation in performance improvement activities.
  • Develops and implements action plans to achieve best practice goals as well as sustainability process for care of patients within specific disease process(s)
  • Maintains outcomes and quality improvement activities by daily or weekly rounding on patients to track individual patient outcomes in collaboration with Nurse Manager, Medical Director, Quality Management Representative, and members of appropriate team.
  • Supports and assist in any clinical research associated with program.
  • Reviews all complaints and exercises discretion and judgement when resolving problems as they arise.
  • Responds to patient and significant others concerns through counseling and education regarding program.
  • Serves as Liaison between Emergency Department and in-patient units unit to facilitate transfers/admissions of identified patient populations.
  • Coordinates with therapists, Care Management, and Rehabilitation/Nursing Homes intake evaluators (as appropriate) to ensure patient transition in a timely and seamless manner.
  • Collaborates with physicians to ensure efficient quality care for all patients.
  • Prepares appropriate reports accurately and within established guidelines and timeframe.
  • Ensures that all communications are accurate and professional.
  • Delegates, supervises, and provides clinical support to staff on prioritizing continuing interventions and establishing treatment goals for patients.
  • Conducts education needs assessments as necessary for all staff involved in care of patients with identified disease processes.
  • Develops educational plans and competency assessments for ongoing education and assessment
  • Conducts in-service meetings, seminars, and conferences for presentation to physicians, staff, and patients regarding prevention, health management and ongoing care.
  • Facilitates development of specific disease(s) processes education program for staff-orientation program and yearly competencies.
  • Participates in on-boarding of medical staff for appropriate education of standards exercised at Capital Health.
