About The Position

The RN Primary Care Nurse provides clinical leadership and day-to-day nursing oversight in an outpatient office serving Beaumont residents with a focus on independent living residents. This role works in close partnership with the LPN to support safe, efficient, resident-center primary care operations; perform assessment, triage, care coordination, and emergency response; support providers and CRNPs; and ensure clinical activities are completed within appropriate scope of practice, competency, organizational standards, and regulatory requirements.

Requirements

  • Current Registered Nurse license in good standing in the applicable state.
  • Current CPR/BLS certification required.
  • Must possess a current, unencumbered, active license to practice as an RN/LPN in this state.
  • Must have current CPR certification.
  • Must be able to read, write, speak, and understand the English language.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess leadership and supervisory ability and willingness to work harmoniously with professional and non-professional personnel.
  • Must possess the ability to plan, organize, develop, implement and interpret the programs, goals, objective, policies, and procedures, etc., that are necessary for providing quality care.
  • Demonstrated ability to effectively use and adapt to a variety of digital tools, software, and systems relevant to the role. This includes the ability to quickly learn new technologies, troubleshoot basic technical issues, and apply technology to improve efficiency, accuracy, and communication.

Nice To Haves

  • Minimum of two years of nursing experience preferred; primary care, ambulatory care, geriatrics, CCRC, long-term care, independent living, or community health experience strongly preferred.
  • Demonstrated experience with outpatient triage, resident assessment, care coordination, wound assessment, chronic disease management, medication follow-up, and response to urgent or emergent resident situations.
  • Ability to lead daily clinical workflow, work effectively with an LPN partner, prioritize competing clinic and resident needs, communicate clearly, and support team-based care.
  • Two years nursing experience preferred.

Responsibilities

  • Coordinate daily primary care operations, resident flow, provider support needs, and prioritization of clinical tasks in partnership with the LPN and clinic team.
  • Perform clinical assessment, telephone and walk-in triage, same-day resident concern evaluation, disposition planning, and escalation of urgent or changing resident conditions.
  • Work collaboratively with the LPN to perform outpatient clinic tasks as needed, including nurse visits, injections, immunizations, wound assessments, dressing changes, vital sign review, medication-related follow-up, resident education, and documentation, consistent with RN scope of practice.
  • Provide clinical leadership, assignment, delegation, guidance, and support to the LPN based on resident acuity, staff scope of practice, competency, clinic priorities, and provider direction.
  • Respond to medical emergencies involving independent living residents, initiate appropriate assessment and interventions within RN scope, activate emergency medical services or internal response protocols as indicated, communicate with providers and leadership, and complete required follow-up documentation.
  • Conduct clinical review of all emergency incidents to evaluate the appropriateness of the response, confirm timely escalation and follow-up, identify opportunities for improvement, and demonstrate sound clinical judgment in decision-making and documentation.
  • Communicate clinical concerns, abnormal findings, and changes in resident status to providers, CRNP, Nurse Care Manager, leadership, families or responsible parties as appropriate, pharmacies, specialists, hospitals, emergency responders, and other care partners.
  • Support care coordination for independent living residents, including high-risk residents, hospital or rehabilitation discharges, chronic disease management, preventive care, medication follow-up, and referrals to internal or external services.
  • Monitor clinical documentation for completeness, accuracy, timeliness, and compliance with organizational standards.
  • Participate in quality improvement, infection prevention, competency validation, policy development, and clinical workflow improvement initiatives.
  • Upholds all HIPAA regulations.
  • Promotes each resident’s optimal functioning in their activities of daily living.
  • Respects and supports each resident’s choices regardless of personal beliefs.
  • Able to manage difficult situations with residents, family and staff.
  • Ensures each resident’s privacy and dignity
  • Is respectful and supporting of the individual resident’s lifestyle and works with those preferences throughout their stay.
  • Demonstrates the ability to receive and transcribe verbal and written physicians orders.
  • Promotes customer service, quality of care, teambuilding, collaboration, accountability, and fiscal responsibility.
  • Demonstrates knowledge Beaumont Employee Handbook and disciplinary process.
  • Responsible for attending staff meetings or getting the information presented during meetings.
  • Responsible for pronouncing the death of a resident and notifying physician, family and funeral home.
  • Coordinates, manages directs and delivers hands on nursing care.
  • Is an educator and clinical resource to staff. Communicates topics to the CRNP for staff education to improve quality of care.
  • Upholds the Beaumont Employee polices.
  • Must be cross-trained in all areas including Skilled Nursing, Personal Care and Wellness. Must be willing to work in all areas as needed.
