About The Position

The Sexual Reproductive Health nurse is responsible for providing indirect and direct nursing care via telephone or in person care in a Federally Qualified Health Center in an urban setting. The SRH nurse will work with our Sexual and Reproductive Health, Women’s Health, and HIV/Infectious Disease department to provide trauma informed, patient centered, evidence-based nursing, with the goal of providing each patient with the care they need, at the time they need it. The SRH nurse will counsel and educate patients around topics relevant to sexual health, provide treatment (following relevant standing orders) and manage a population of patients requiring management for abnormal test results.

Requirements

  • Graduate of an accredited school of nursing required; BS in nursing preferred.
  • Must be licensed as a RN by the Massachusetts State Board of Registration.
  • BLS required.
  • Two years full time, or part time equivalent of nursing experience, with at least one year of telephone triage experience in an ambulatory health care or family practice setting preferred.
  • Must have experience working in an EMR environment.

Responsibilities

  • Actively participates in managing reproductive health screenings, outreaching to patients to discuss pertinent health issues, and supporting primary care teams in delivering evidence-based care to vulnerable populations.
  • Utilizes care in communications to foster the caller’s trust & confidence.
  • Arranges for the use of Interpreter staff as required based on caller’s primary language needs.
  • Demonstrates competent clinical/professional judgment in addressing sensitive health and social related topics.
  • Assesses all calls using three specific criteria: 1.) clinical priority/acuity, 2.) the need for self-care guidance, 3.) the need for anticipatory guidance based upon the patient’s health, behavior and developmental level. Assesses presenting signs/symptoms prioritizing urgency based on assessment and use of protocols.
  • Follows-up on abnormal labs and other patient requests by communicating with Team Nurses and Providers to facilitate the appropriate care.
  • Conducts billable in-person (and telehealth) visits related to pregnancy tests, contraception, STI screening and treatment, pre-exposure prophylaxis and other Sexual Reproductive Health patient needs.
  • Follows protocol and documents calls, visits and encounters in the EHR, following the FHCW documentation policy.
  • Under the direction and supervision of a provider and utilizing approved standing orders and clinical protocols, performs independent nursing visits for cervical cancer screening follow-up, contraception counseling, pregnancy testing and counseling, STI prevention, education, and treatment, pre- and post-procedure colposcopy counseling, and ongoing management of patients requiring surveillance for abnormal gynecologic findings.
  • These additions align with the existing duties of abnormal Pap tracking, Breast and Cervical Cancer follow-up, preventive care, patient outreach, and nurse-led patient education described in the job description.
