About The Position

The RN Per Visit is responsible for managing patient care within the home health setting. This includes evaluating clinical needs, developing and implementing care plans, coordinating with interdisciplinary teams, and ensuring accurate documentation. The role requires adherence to agency standards, communication with physicians and other providers, and timely completion of all necessary paperwork, including admissions and discharges.

Requirements

  • Successful passage of VNA pre-hire medication test
  • Current RN license in Mass. and other related credentials to meet specialty.
  • Minimum one (1) year of nursing experience required or 6 months of nursing experience with a BSN.
  • CPR certification required.

Responsibilities

  • Maintain annual HHRG values at the National Standard as appropriate.
  • Utilize HHRG values to establish visit frequency and to establish appropriate interdisciplinary utilization.
  • Ensure annual outcomes meet National Benchmarks.
  • Refer to community resources as appropriate.
  • Evaluate the clinical needs of the patient; initiate, develop, implement and make necessary revisions to the patient's plan of care; address all problems in the plan of care and document accordingly.
  • Participate in inter-disciplinary patient case conferences; evaluate effectiveness of established plan of care.
  • Instruct and provide documented supervision of the Home Health Aide plan of care every 14 days.
  • Notify MD/other involved care providers of significant changes in the patient’s condition.
  • Complete admission and other paperwork on a timely basis.
  • Record narrative documentation to reflect skilled care. Documentation is clear, concise and addresses specifics of teaching, direct care and response of patient / caregiver.
  • Obtain and clarify valid MD orders for all care delivered.
  • Plan and coordinate discharges appropriately and inform patient/MD and record disposition of patient in the discharge summary. Complete all appropriate discharge paperwork.
  • Perform all other duties as assigned.
