The Per Diem RN is responsible for assessing and identifying patient/family needs, utilizing the nursing process, coordinating the Plan of Care with the Interdisciplinary Group (IDG), and providing palliative and supportive care to the patient/family unit. This role involves completing clinical admissions, explaining services, identifying admission objections, educating patients/families, obtaining consents, and initiating skilled nursing interventions. The RN also acts as a representative at assigned facilities, facilitates referrals, communicates with Admissions, obtains medical record information, develops relationships with case managers and physicians, and provides education on available services. Additionally, the RN provides care for GIP patients in hospitals, including daily visits, professional management, documentation, and efficient discharge. Responsibilities include managing direct patient care, coordinating the Plan of Care, educating patients and families, reporting changes in patient condition, evaluating hospice referrals, attending patient deaths, providing bereavement interventions, and supervising other nursing team members. During emergencies, the RN may be required to report to a designated company location to ensure continuity of services.
