About The Position

The RN Navigator is responsible for advancing exceptional surgical care through coordination, support, education, and advocacy. The Navigator collaborates with key stakeholders to plan, organize, and manage activities of the Orthopedic, Spine, and Bariatric Surgical Programs. Maintains compliance and certification with the American Society of Metabolic and Bariatric Surgery (ASMBS), Joint Commission Orthopedic Advanced Certification, and Joint Commission Spine Certification. The Navigator coordinates resources into a seamless model of access, care, and support that benefits patients, families, and physicians. The Navigator addresses patient needs throughout the surgical continuum to reduce gaps in care processes. Implements and promotes strategies to ensure accomplishment of positive patient outcomes.

Requirements

  • BSN or degree in healthcare related field.
  • Registered Nurse (RN)
  • AHA Basic Life (CPR) Certification
  • Acute care clinical, quality, or informatics experience.
  • Meditech, Microsoft applications including Outlook, Word, Excel, PowerPoint.
  • Requires effective use of problem solving, negotiating, conflict management, team leading and teambuilding skills.
  • Must possess ability to communicate well in written and verbal forms with others, both internally and externally.
  • Must have a thorough knowledge of health system organizations, administration, policies and procedures.

Nice To Haves

  • Master’s Degree preferred.
  • Certification in specialty preferred

Responsibilities

  • Establishes close relationships with and serves as primary point of contact for orthopedic, spine and bariatric patients and provider offices.
  • Serves as liaison for patients and their care teams.
  • Responsible for follow up with patients, monitoring progress, coaching, and providing education though the completion of the surgical journey.
  • Communication is a combination of in person, by phone, email, video conference, and within the EMR system.
  • Responsible for identifying concerns through the surgical continuum and facilitates timely communication and collaboration between office practice, scheduling, preoperative, intraoperative and postoperative phases, and post-acute care teams.
  • Oversees data collection, patient reported outcomes, and abstracting of information into reporting tools and patient management systems.
  • Assists with the development and implementation of evidence-based protocols and programs.
  • Responsible for required documentation for Joint Commission Advanced Orthopedic and Spine Certifications and Metabolic and Bariatric Surgery Accreditation and Quality Program
  • Facilitates Center of Orthopedic Excellence, Spine, and Bariatrics Committee meetings.
  • Contributes to the strategic management and planning for the Joint Commission Advanced Orthopedic and Spine Certifications and Metabolic and Bariatric Surgery Accreditation and Quality Program surveys.
