Requirements

• Current registered nurse licensure to practice in the state where the role is based.
• Five years of RN experience.
• Previous experience in a home care setting.
• Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements.

• Two years of management or supervisory experience in home health.

Responsibilities

• Provide clinical supervision to ensure patient care aligns with professional standards, agency policies, and regulatory requirements.
• Plan, coordinate, and monitor patient care to support high-quality outcomes and consistent service delivery.
• Supervise, support, and coach clinical personnel to maintain care excellence and encourage staff development.
• Facilitate clear communication among patients, families, referral sources, and care team members.
• Drive quality improvement initiatives and meet departmental performance goals.
• Model professional leadership, service excellence, and organizational values.
• Maintain compliance through required training, accurate documentation, and cooperation with audits.
• Promote a safe work environment by implementing and enforcing workplace safety protocols.
