Franciscan Senior Health and Wellness PACE is seeking a Required Nurturer (RN) to join their team. This role involves providing exceptional patient care and promoting health and wellness within the community. The PACE program focuses on providing unmatched, individualized, and joyful care to seniors, enabling them to live in their own homes by offering comprehensive services including care, nutrition, rehabilitation, transportation, and supportive services. Requirements
- Associate's Degree- Nursing/Patient Care - Required
- Registered Nurse (RN)- State Licensing Board - Required
- Basic Life Support Program (BLS) - American Heart Association - Required
- 1 Year- Nursing/patient care with frail/elderly in an in-patient nursing or home health setting Experience- Required
Nice To Haves
- Bachelor's Degree- Nursing/Patient Care- Preferred
- 2 Years- Nursing Experience- Preferred
Responsibilities
- Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care.
- Maintain accurate, detailed reports and records.
- Administer medications to patients and monitor patients for reactions or side effects.
- Modify patient treatment plans as indicated by patients' responses and conditions.
- Prepare patients for and assist with examinations or treatments.
- Monitor, record, and report symptoms or changes in patients' conditions.
