The RN, Case Manager is responsible for assessing and identifying patient/family needs, utilizing the nursing process, coordinating the Plan of Care with the Interdisciplinary Team (IDT), and providing clinical, palliative and supportive care to the patient/family unit in order to keep the participant in their home environment as long as possible. This role involves providing reassurance on the phone to patients and families, assisting in finding solutions to their questions, and recognizing the need for an in-person visit. The RN will coordinate in-person visits when needed or requested, utilize appropriate support/expert resources to resolve complex situations, and document patient/family contact information in the EMR. They will complete initial and semi-annual assessments, explain services, and address questions regarding patient needs, fears, and physical limitations in an empathetic and compassionate manner. The position also involves providing information to Physicians and other IDT members, initiating the Plan of Care, and performing skilled nursing interventions to enhance prevention, prevent complications, alleviate symptoms, and maximize comfort. The RN acts as the Company representative at assigned facilities, works closely with referring hospitals and physicians, and communicates frequently with the IDT using SBAR. They will also supervise patient care provided by Community Health Workers and Home Health Aides as requested. During emergencies, the RN may be required to report to work at a designated company location to ensure continuity of services, which may include reporting ahead of schedule and staying overnight.
