About The Position

Delivers coordinated nursing care for a patient or assigned group of patients according to established standards of care and the nursing process. Supervises and directs the activities of assigned nursing staff at various levels, and coordinates care with other disciplines using critical thinking, professional and supervisory discretion, and independent judgment. This is a full-time, day-shift position in Primary Care. Adventist Health Sonora is a healthcare provider serving Tuolumne and Calaveras counties and surrounding areas, with a hospital, medical offices, cancer care, and other services.

Requirements

Registered Nurse (RN) licensure in the state of practice.
CPR certification or Basic Life Support (BLS) certification from an approved vendor per Adventist Health policy.
Basic Life Support (BLS) certification from an approved vendor per Adventist Health policy is required for the facility.

Nice To Haves

Bachelor’s Degree in Nursing (BSN).
Acute care facility experience.

Responsibilities

Collect relevant data pertinent to patients’ health or situation.
Analyze assessment data to determine diagnoses and care issues.
Develop plans prescribing interventions to attain outcomes; implement plans, coordinate care delivery, promote health and a safe environment, and evaluate progress toward outcomes.
Identify outcomes for patients or their situations.
Collaborate with patients, families, and healthcare providers to provide care in a safe, healing, humane, and caring environment.
Provide learning opportunities and health information to patients, family members, and team members.
Participate in discharge planning to provide continuity of care.
Delegate appropriately and coordinate the duties of healthcare team members.
Supervise and direct assigned nursing staff.
Perform other job-related duties as assigned.
