Capital Health is seeking an RN for an Outpatient Primary Care Practice. This is a full-time, day shift position. Requirements

  • Associate’s degree or graduation from an accredited school of nursing.
  • NJ or PA license based on primary state office location or multi-state compact license required.
  • Must have applied for second out of state RN license (NJ or PA) within three months from date of hire.
  • One year experience in an acute, urgent care or outpatient setting.
  • AHA BLS - Healthcare Provider

Responsibilities

  • Triages and assesses patients, determining clinical priority by evaluating their condition, medical history, and current status through screening, interviews, medical record review, and physician notes.
  • Performs and documents parts of the physical assessment, including chief complaint and vital signs.
  • Obtains and records patient medical history as needed.
  • Discusses and reviews patient prognosis with medical staff.
  • Reviews and coordinates patient medical phone calls, assesses patient status, and schedules treatment according to established protocol.
  • Reviews medication refill requests, laboratory, and other test results daily, prioritizing them for LIPs according to established protocol.
  • Performs nursing care and treatment modalities appropriate for individual patients.
  • Assists LIP with procedures, performs EKGs, removes sutures, cleans wounds, applies dressings, obtains specimens and cultures, and labels patients and specimens according to CH policy.
  • References policies and procedures to ensure Capital Health best practices are followed.
  • Administers medications accurately and safely.
  • Documents patient records according to established protocol.
  • Performs and demonstrates technical skills specific to the assigned ambulatory setting.
  • Calls in prescriptions to pharmacies under the direction of the LIP.
  • Completes competency checklist with preceptors.
  • Recognizes important changes in patient condition and reacts appropriately in the ambulatory setting.
  • Acknowledges and includes the patient's family during communication with the patient, as appropriate and designated by the patient.
  • Supports principles from Capital Health core mission (quality care, compassion, dignity, and comprehensive continuum of care).
  • Seeks interpretation or understanding of the illness, pain, fear, anxiety, or other stressful emotions.
  • Prepares and maintains thorough, clear, and concise documentation in patient charts.
  • Documents nursing actions, type and effect of treatments, patient responses, and other required information.
  • Performs Quality Assurance (QA) activities as defined by the ambulatory setting.
  • Documents quality control for waived testing and sterilization of instruments as required by established policies and procedures.
  • Oversees other quality and regulatory initiatives documentation and takes appropriate action to remedy any discrepancies.
  • Demonstrates a willingness to adapt to changing department demands.
  • Demonstrates willingness to assist co-workers during periods of heightened patient care activity.
  • Performs other duties as assigned and may be assigned to other Capital Health Medical Group Practices as needed.
