About The Position

Capital Health is seeking an RN for their Outpatient Primary Care practice in Bordentown, NJ. This is a full-time, day shift position. The role involves triaging and assessing patients, determining clinical priority, performing and documenting physical assessments (including chief complaint and vital signs), and obtaining medical history. The RN will coordinate patient medical phone calls, assess patient status, and schedule treatment according to protocol. They will also review medication refill requests and laboratory/test results, prioritizing them for LIPs. The position requires performing nursing care and treatment modalities, assisting LIPs with procedures, performing EKGs, removing sutures, cleaning wounds, applying dressings, obtaining specimens and cultures, and ensuring adherence to Capital Health best practices. Administering medications accurately and safely, documenting patient records, performing technical skills specific to the ambulatory setting, and calling in prescriptions under LIP direction are also key responsibilities. The RN will recognize and react to changes in patient condition, include patient families in communication as appropriate, and support Capital Health's core mission. Thorough and concise documentation in patient charts, including nursing actions, treatment effects, and patient responses, is essential. The role also includes performing Quality Assurance (QA) activities, documenting quality control for waived testing and instrument sterilization, and overseeing other quality/regulatory initiatives. The position requires adaptability to changing department demands and willingness to assist coworkers. Other duties as assigned may include assignments to other Capital Health Medical Group Practices.

Requirements

  • Associate’s degree or graduation from an accredited school of nursing.
  • NJ or PA license based on primary state office location or multi-state compact license required.
  • Must have applied for second out of state RN license (NJ or PA) within three months from date of hire.
  • One year experience in an acute, urgent care or outpatient setting.
  • AHA BLS - Healthcare Provider

Responsibilities

  • Triages and assesses patients and determines the clinical priority by assessing patient's condition, medical history, and present status by screening and interviewing patient, by reviewing medical records, and by reviewing physician notes.
  • Performs and documents part of the physical assessment including but not limited to chief complaint and vital signs.
  • Obtains and records patient medical history as needed.
  • Discusses and reviews patient's prognosis with medical staff.
  • Reviews and coordinates patient's medical phone calls, assesses the patient's status, and schedules treatment according to established protocol.
  • Reviews medication refill requests, laboratory and other test results daily and prioritizes them for LIPs according to established protocol.
  • Performs nursing care and treatment modalities appropriate for individual patients.
  • Assists LIP with procedures, performs EKG, removes sutures, cleans wounds, applies dressings, obtains specimens and cultures while labeling patients and specimens according to CH policy.
  • References policies and procedures to assure Capital Health best practices are followed.
  • Administers medications accurately and safely.
  • Documents patient's records according to established protocol.
  • Performs and demonstrates technical skills specific to assigned ambulatory setting.
  • Calls in prescriptions to pharmacies under the direction of the LIP.
  • Completes competency checklist with their preceptors.
  • Recognizes important changes in patient condition and reacts appropriately in ambulatory setting.
  • Acknowledges and includes patient's family during communication with the patient, as appropriate and as designated by ambulatory patient.
  • Supports principles from Capital Health core mission (quality care, compassion, dignity and comprehensive continuum of care).
  • Seeks interpretation or understanding of the illness, pain, fear, anxiety or other stressful emotion.
  • Prepares and maintains thorough, clear, and concise documentation in patient's charts.
  • Documents nursing actions, type, and effect of treatments, patient responses, and other required information.
  • Performs Quality Assurance (QA) activities as defined by ambulatory setting.
  • Documents quality control for waived testing and sterilization of instruments as required according to established policies and procedures; Oversees other quality and regulatory inititatives documentation takes appropriate action to remedy any discrepancies.
  • Possesses a willingness to adapt to changing department demands.
  • Demonstrates willingness to assist co-workers during periods of heightened patient care activity.
  • Performs other duties as assigned and may be assigned to other Capital Health Medical Group Practices as needed.
