About The Position

Home Care Plus is seeking an experienced RN or LPN to serve as a Community Nurse Liaison and Care Coordinator in the Charleston, SC area. This role focuses on making a significant impact outside of traditional bedside nursing by combining clinical expertise, relationship-building, community outreach, and care coordination. The ideal candidate will help local families navigate their care needs and act as a trusted resource for healthcare professionals in the Charleston area. This position offers flexibility and autonomy, allowing the nurse to manage their schedule while building community relationships and ensuring families access quality care. It is not a hospital floor position and does not involve working nights, weekends, or holidays.

Requirements

  • Holds an active South Carolina RN or LPN license
  • Has experience in home care, hospice, case management, discharge planning, senior care, rehabilitation, or a related healthcare field
  • Knows the Charleston, Mount Pleasant, and surrounding Lowcountry healthcare community
  • Is comfortable building professional relationships and networking within the community
  • Communicates confidently with physicians, social workers, case managers, and families
  • Is self-motivated, organized, and able to work independently
  • Wants to grow with a locally owned company that values initiative and leadership

Nice To Haves

  • 5+ years of nursing experience
  • Geriatric experience required
  • Experience with business development, community outreach, admissions, or care coordination
  • Home care industry experience strongly preferred

Responsibilities

  • Conduct in-home assessments and develop personalized care plans
  • Meet with clients and families to understand their needs and recommend solutions
  • Build and maintain relationships with hospitals, physician offices, rehabilitation centers, senior living communities, hospice organizations, and other referral partners
  • Represent Home Care Plus at community events, networking functions, and healthcare meetings
  • Support caregiver recruitment, onboarding, and quality assurance visits
  • Serve as a clinical resource for clients, families, caregivers, and referral partners
