About The Position

As a nonprofit community mental health center serving southeastern Jackson County, Missouri, we are committed to helping individuals impacted by mental illness and substance use find stability, hope, and whole-person care. Our Health Care Home team supports preventive care, chronic condition management, and health education for individuals with complex physical and behavioral health needs, including children and adolescents. As a Nurse Care Manager, you will play a central role in coordinating care, improving health outcomes, and supporting clients through proactive outreach and structured engagement. This is a highly collaborative role, working closely with Community Support Specialists and interdisciplinary care teams.

Requirements

  • Graduation from an accredited nursing program (LPN or RN)
  • Active, unrestricted Missouri nursing license
  • Valid driver’s license, proof of insurance, and reliable transportation
  • Comfort working in electronic health records and standard Microsoft Office tools
  • Ability to work both independently and as part of a highly collaborative team

Nice To Haves

  • Pediatric or adolescent experience strongly preferred (experience working with children ages 5+)
  • Experience working in mental health, behavioral health, or substance use settings preferred (2+ years ideal)
  • Strong communication, engagement, and client-centered care skills

Responsibilities

  • Collaborate with interdisciplinary teams to develop and implement client-centered care and wellness plans
  • Provide structured outreach and engagement to maintain consistent client contact and support continuity of care
  • Conduct health assessments, screenings (quarterly/annual), and wellness monitoring
  • Support pediatric and adult clients with chronic medical conditions and behavioral health needs
  • Monitor hospital admissions/discharges and complete medication reconciliation
  • Coordinate care with external healthcare providers to ensure continuity and quality of services
  • Deliver individualized health education and wellness support to clients and families
  • Maintain accurate and timely documentation in alignment with program productivity expectations
  • Track and support performance measures related to physical health outcomes
