This position coordinates and oversees the medical care and treatment of a caseload of individuals with intellectual and developmental disabilities. Works collaboratively with physicians, interdisciplinary teams, individuals in services, providers, and families to promote positive outcomes. Applies clinical knowledge to assess individuals and provide consultation and training. Assesses plans, implements, coordinates, monitors, and evaluates services required to support the individual’s health needs.

Requirements

  • RN with current Colorado license.

  • Experience as a Registered Nurse working with individuals with intellectual and developmental disabilities.
  • Knowledge and experience of physical conditions and treatment protocols for conditions of the IDD population.
  • Nurse case management experience.

Responsibilities

  • Provides delegation and consultation to unlicensed providers, families, and Support Inc staff.
  • Provides medical consultation to interdisciplinary team members.
  • Teaches and trains providers, families, and staff in: Medication Administration (QMAP) and MRB training, Individual specific medical documentation, Evaluate signs of illness or dysfunction that warrant medical or nursing intervention, Basic skills required to meet the health needs of the individual., Utilizing specialized medical equipment.
  • Applies knowledge of nursing practices, state and federal regulations, guidelines, and procedures to ensure compliance with home and community-based services.
  • Completes and documents evaluations and assessments to identify training and support needs to ensure continuity of care.
  • Develops care plans, reviews medical documentation, and health tracking.
  • Develop effective working relations and cooperate across teams throughout the entire case management process.
  • Help individuals receiving services and their caretakers make informed decisions by acting as their advocate regarding their clinical status and treatment options.
