We have an excellent Full-Time opportunity for a dynamic Home Health Registered Nurse (RN) to care for our residents in the comfort of their own homes. Must be licensed in the State of Missouri as a Registered Nurse, have excellent communication and computer skills as well as reliable transportation. Requirements
- Current Registered Nurse (RN) license in the State of Missouri
- CPR certification required
- Minimum of one (1) year experience in an adult medical/surgical unit
- Excellent communication skills
- Excellent computer skills
- Reliable transportation
Nice To Haves
- Six (6) months of home care experience preferred
Responsibilities
- Assist the men with scheduling and coordination of health care specialists
- Accompany Jesuits to physician or specialist appointments as needed
- Report changes in condition and relevant health updates to the nurse on duty, Care Coordinator Nurse, and Jesuit leadership
- Manage admissions and readmission, including communication of condition, treatments, and medications
- Ensure care plans are accurately and promptly communicated to the Care Coordinator Nurse and/or floor nurse on duty
- Assume full responsibility for assigned cases
- Complete documentation, charting, and planning in accordance with prescribed procedures
