The RN Case Manager in this role is responsible for providing expert nursing and coordination of care for pediatric patients within a specialty population and their families. This includes conducting patient assessments, writing plans of care and developing educational programs. Also, includes program development to better meet the comprehensive needs of this population.

Requirements

  • 2+ years clinical experience related to program specialty required
  • Bachelor's Degree in Nursing required
  • BLS-Basic Life Support Healthcare Provider - American Heart Association/American Red Cross/National Safety Council required
  • License RN-Registered Nurse (30) - State of Wisconsin required

  • Experience in case management and program development preferred

Responsibilities

  • Understands and satisfies the needs of the neonatal, infant, toddler, pre-school, school-age, adolescent and/or adult patient in regard to their growth and development process.
  • Develops, implements, manages, markets and evaluates a program plan.
  • Synthesizes and evaluates the individualized patient/family responses along the continuum of care; makes changes in treatment within the scope of practice.
  • Negotiates systems, plans and directs the implementation of change related to program plans and standards.
  • Uses case management as a strategy to assist in managing a population; collaborates with the multidisciplinary team in the establishment of goals and implementation of the plan of care; identifies priorities and realistic outcomes for intervention; identifies and communicates potential solutions to variances; implements identified solutions.
