About The Position
MDS Coordinator/ Nurse Assessment Coordinator (RN) Join Our Team at White Oak of Charlotte! White Oak of Charlotte is looking for a dedicated MDS Coordinator/ Nurse Assessment (RN) to join our compassionate team. Requirements
- Active South Carolina Registered Nurse (RN) license
- Graduate of an accredited school of nursing
- Minimum one year of long-term care experience
- Working knowledge of Medicare, PPS, PDPM, and Managed Care reimbursement
- Strong understanding of federal and state long-term care regulations
- Excellent assessment, organizational, communication, and leadership skills
- Experience with electronic medical record (EMR) systems
Nice To Haves
- Previous MDS Coordinator experience preferred
Responsibilities
- Coordinate the interdisciplinary resident assessment and care planning process
- Complete accurate and timely MDS assessments (admission, readmission, quarterly, annual, significant change, and discharge)
- Complete Care Area Assessments (CAAs) and individualized care plans within required regulatory timelines
- Review and update care plans to reflect residents' changing needs and goals
- Lead and participate in care plan meetings and family conferences
- Communicate resident care plan updates with families, physicians, and the interdisciplinary team
- Manage Medicare PPS, PDPM, and Managed Care assessment schedules
- Ensure clinical documentation supports assessment accuracy and reimbursement requirements
- Assist with state and federal surveys and maintain regulatory compliance
- Stay current on MDS regulations, reimbursement guidelines, and industry best practices
- Educate staff on documentation and resident assessment processes
- Maintain accurate resident records using electronic medical record (EMR) systems
- Perform additional duties as assigned by the Director of Nursing
