The RN works in a Level III NICU with 26 beds and typical nurse-to-patient ratios of 1:2 and 1:1. The unit cares for premature infants, including patients with sepsis, and surgical cases such as gastroschisis and EA/TEF. Patients may require nasal CPAP and, rarely, ventilation or oscillators. The NICU attends high-risk deliveries in Labor and Delivery. Respiratory therapy is available in the unit 24 hours a day, and a neonatologist is in the unit 24 hours a day. The RN monitors patients in an open-bay unit with telemetry and must be experienced in titrating dopamine, dobutamine, Versed, and fentanyl. Equipment includes oscillators, nasal CPAP, rare ventilators, giraffe bassinets, atom incubators, and radiant warmers. Nurses may float first to the Nursery or Pediatrics, a sister unit with 12 beds and generally low-acuity patients, such as those with RSV or pneumonia. Shifts are 12 hours, starting at 7:00 or 7:30. Nurses self-schedule by seniority, with the manager balancing schedules. The role requires every-other-weekend work and holiday coverage for one major and one minor holiday. There is no call or standby requirement. Orientation includes three shifts on the unit with a preceptor, plus six hours in the Nursery and six hours in Pediatrics. The scrub color is navy.
