About The Position

Sanford Health is seeking a Nurse Navigator for the Pediatric Hematology and Oncology Clinic within the Roger Maris Cancer Center. This role involves triaging new referrals, onboarding new patients, working with tumor boards, collecting program data, performing telephone triage, managing central lines, providing patient education, and coordinating care for oncology and hematology patients. There is an opportunity for cross-training to the pediatric infusion clinic. The clinic cares for children, adolescents, and young adults with oncology and non-oncology diagnoses. The schedule consists of 8-hour day shifts from 7:45 AM to 5:00 PM, with no evening shifts, weekends, or holiday commitments.

Requirements

  • ADN or BSN required.
  • Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
  • Obtains and subsequently maintains required department specific competencies and certifications.
  • Graduate from an accredited nursing program, including, but not limited to, those accredited by the Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).
  • ND, SD and MN RN licenses required (SD and MN can be obtained after start).

Nice To Haves

  • Bachelor's degree in Nursing strongly preferred in Bemidji, Marshfield, and Rapid City.
  • Required in Fargo and Sioux Falls (Sanford USD Medical Center and Clinics).
  • Required in Bismarck or enrolled and starting a Bachelor’s degree program within one year of hire with a completion date within three years upon date of hire into position.
  • Required in Fargo Network and Sioux Falls Network or employees hired after January, 2025, who do not possess a Bachelor’s degree in the required field, leadership may consider an educational plan with proven continuous action toward achieving a Bachelor’s degree in Nursing within five years of hire into position.
  • Approval from the Nursing Executive for the market is required in order to waive the bachelor degree requirement (at time of hire).
  • Additionally, for Rural Health Network facilities, for current employees, leadership may have considered acceptable qualifications and work experience equivalency based on facility size, rural market, and business need, prior to new Sanford education requirements.
  • Minimum of two years’ clinical experience preferred.
  • Strong communication skills are required.
  • Self-motivated, working with little direction.
  • Ability to facilitate groups, multidisciplinary teams, and implement best practices when necessary.

Responsibilities

  • Integrates health care for patients/clients across the continuum of care.
  • Collaborates with healthcare team and other professionals to coordinate and ensure timely sequencing of care.
  • Supports patients/clients and other family members by removing barriers of care.
  • Seeks to evaluate outcomes based on an integration among established clinical, financial and utilization data.
  • Functions in structured and unstructured health care settings.
  • Utilizes independent nursing judgment when integrating health care.
  • Applies appropriate age-related patient care protocols relating to the physical and psychological needs of pediatric, adolescent, adult and geriatric patients.
  • May serve as a key link between providers, clinics, insurance concerns, communication, inpatient floors and outside facilities.
  • Establishes and maintains good interpersonal relationships.
  • Works with growth and development needs of client populations in clinical area.
  • Coordinates, facilitates and negotiates with others.
  • Monitors, evaluates and trends patient responses utilizing structured care methodologies.
  • Works closely with ancillary and support services.
  • Solves problems, identifies and implements process improvements when needed.
  • Functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure.
