About The Position

The Orthopedic Nurse Navigator is a Registered Nurse with orthopedic and Total Joint-specific clinical skills and comprehensive knowledge of Total joint arthroplasty, pre-operative and post operative treatment modalities, and preoperative medical optimization. Responsible for patient coordination, episode of care coordination, risk assessment, post discharge follows up and organization with priority focus in the high-risk patient to decrease preventable re-admissions and postoperative complications and improve quality of care and patient satisfaction. The Orthopedic Nurse Navigator will demonstrate commitment to quality patient care, implement creative and innovative ways to meet diverse, complex patient needs and ensure best practices and standard of care for total joint patients are followed. Serves as clinical advocate and educator for patients. The Orthopedic Nurse Navigator will build and maintain relationships with orthopedic surgeons, APP's, office surgery schedulers and staff to provide reliable consistent communication regarding high-risk total joint patients. The position is associated with a significant level of autonomy; therefore, the individual must be highly motivated and able to work independently. Demonstrates skill as a clinician and educator.

Requirements

Bachelor's Degree in Nursing - Required
3 years clinical experience working with surgical patients - Required
5 years clinical experience in a hospital or ASC - Required
Clinical Other\BCLS - Basic Life Support - Required
Nursing\RN - Registered Nurse - State Licensure And/Or Compact State Licensure - Required

Nice To Haves

Master's Degree in Nursing - Preferred
3 years clinical experience with Total joints/ orthopedics/ geriatrics/ OR/ Nure Navigator - Preferred
Nursing\ONC - Orthopedic Nurse Certified - Preferred

Responsibilities

Pre-Surgical introduction to HH and initiation of seamless care by connecting with surgeon's clinics, surgery schedulers and staff to identify high risk patients' multiple weeks prior to surgery.
Contact high risk patients to assess needs, coordinate appointments for in-person total joint class and any other identified needed appointments prior to surgery for optimization.
Post -surgical follow up by rounding and reconnecting with patients during their stay at the Hospital, assist with needs and requests.
Ensure patients are on complex care post op order sets.
Continued follow up with post operative phones calls on a set cadence once patients are discharged from the hospital to help patients be compliant with plans of care for optimal outcomes.
Teach in-person total joint class at more than one primary hospital location.
Obtain patients signatures on the pre/post expectation form.
Provide important needed contact information.
Answer questions and concerns that are in the Orthopedic nurse navigator expertise and then coordinate all other questions to appropriate team members to obtain needed answers.
Partners with quality department, service line leader, program leader and surgeons to develops measures and reports for outcomes to assure continuous improvement.
Maintains appropriate and timely documentation through medical record entries, daily logs, and monthly statistics.
Prepares and maintains required documentation on every patient.
Tracks individual patient activities related to the plan of care.
Present current program standings when needed for service line and surgeon leaders.
Provides education to medical and hospital staff of inpatient and outpatient programs, including appropriate use of discharge planning, and complex patient needs and directs to appropriate available resources.
Performs other duties as assigned.
