About The Position

Sanford Health, the largest rural health system in the United States, is dedicated to transforming the health care experience and providing access to world-class health care in America’s heartland. This role involves integrating health care for patients and clients across the continuum of care, collaborating with healthcare teams to ensure timely sequencing of care, and supporting patients and their families by removing barriers to care. The position requires evaluating outcomes, functioning in various healthcare settings, and utilizing independent nursing judgment. It also involves knowledge of age-related patient care protocols for pediatric, adolescent, adult, and geriatric patients. The navigator may act as a liaison between providers, clinics, insurance, inpatient floors, and outside facilities, requiring strong interpersonal and communication skills. The role involves coordinating, facilitating, and negotiating with others, monitoring patient responses, and working closely with ancillary and support services. Problem-solving, identifying process improvements, and facilitating groups and multidisciplinary teams are also key aspects of this position. The nurse navigator functions within the scope and standards of nursing practice as outlined by the Nurse Practice Act and the American Nurses Association.

Requirements

Bachelor's degree in Nursing strongly preferred in Bemidji, Marshfield, and Rapid City. Required in Fargo and Sioux Falls (Sanford USD Medical Center and Clinics). Required in Bismarck or enrolled and starting a Bachelor’s degree program within one year of hire with a completion date within three years upon date of hire into position.
Approval from the Nursing Executive for the market is required in order to waive the bachelor degree requirement (at time of hire).
Graduate from an accredited nursing program, including, but not limited to, those accredited by the Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).
Currently holds an unencunbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
Obtains and subsequently maintains required department specific competencies and certifications.

Minimum of two years’ clinical experience preferred.
For Rural Health Network facilities, for current employees, leadership may have considered acceptable qualifications and work experience equivalency based on facility size, rural market, and business need, prior to new Sanford education requirements.

May serve as a key link between providers, clinics, insurance concerns, communication, inpatient floors and outside facilities.
Establishes and maintains good interpersonal relationships.
Works with growth and development needs of client populations in clinical area.
Coordinates, facilitates and negotiates with others.
Monitors, evaluates and trends patient responses utilizing structured care methodologies.
Works closely with ancillary and support services.
Solves problems, identifies and implements process improvements when needed.
Facilitates groups, multidisciplinary teams, and implements best practices when necessary.
Functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure.
