This role acts as a patient advocate, guiding patients through the oncology care system. The RN Navigator establishes contact with newly diagnosed cancer patients and/or their family members or caregivers. They understand clinical care options and direct patients to healthcare services within the organization, at outside facilities, and within the community for timely treatment and survivorship. The position actively identifies and addresses population and patient-specific barriers to care that might prevent timely and appropriate treatment. The RN Navigator connects patients with resources, healthcare, and support services in their communities. This role supports the physician and interdisciplinary team in facilitating patient care with the objective of enhancing the quality of clinical outcomes and patient satisfaction. The role also implements day-to-day care management interventions for identified clinic patients.
