About The Position

White Oak Kings Mountain is seeking a highly organized, detail-oriented, and compassionate RN MDS Coordinator / Nurse Assessment Coordinator (NAC) to join their exceptional team. In this vital role, you will assess residents' needs and strengths through the federally and state-approved MDS assessment process. The Nurse Assessment Coordinator develops individualized, functional Plans of Care that address each resident's physical, emotional, and psychosocial needs while supporting quality outcomes and regulatory compliance.

Requirements

• Current, active Registered Nurse (RN) license in North Carolina
• Strong organizational and time management skills
• Excellent communication and teamwork abilities
• Ability to work independently while collaborating effectively with an interdisciplinary team

Nice To Haves

• Experience with MDS assessments and care planning preferred
• Knowledge of Medicare, Medicaid, and long-term care regulations preferred

Responsibilities

• Coordinate and complete MDS assessments in accordance with federal and state regulations
• Accurately assess resident needs, strengths, and clinical status
• Develop and maintain individualized Plans of Care
• Collaborate with interdisciplinary team members to ensure quality resident outcomes
• Monitor assessment schedules and ensure timely submissions
• Assist with care planning and quality improvement initiatives
• Maintain compliance with CMS, state, and facility requirements
• Support reimbursement optimization through accurate documentation and assessment practices
