About The Position

Chemik Healthcare Services is seeking a compassionate, experienced Registered Nurse (RN) or Licensed Vocational Nurse (LVN) who is fluent in Hindi and English to provide exceptional home health care to our diverse patient population. Requirements

• Current Texas RN or LVN license in good standing.
• Fluent in both Hindi and English (required).
• Current CPR/BLS certification.
• Valid driver's license, reliable transportation, and current auto insurance.
• Excellent assessment, communication, and organizational skills.
• Ability to work independently while providing compassionate, patient-focused care.

Nice To Haves

• Minimum of 2 years of home health nursing experience preferred.
• Knowledge of Medicare home health regulations and documentation requirements is preferred.

Responsibilities

• Perform comprehensive patient assessments and develop individualized plans of care.
• Provide skilled nursing services in accordance with physician orders.
• Administer medications, treatments, and wound care as prescribed.
• Educate patients and family members on disease management, medications, and home care.
• Monitor patient progress and communicate changes to the physician and interdisciplinary team.
• Maintain accurate, timely, and compliant clinical documentation.
• Coordinate care with therapists, physicians, caregivers, and other healthcare professionals.
• Ensure compliance with Medicare, state, and agency regulations.
