RN / LVN Pediatric Home Health Nurse
About The Position
Angels of Care Pediatric Home Health is seeking compassionate Registered Nurses (RN) and Licensed Vocational Nurses (LVN) to provide direct, one-on-one medical care to infants, children, and/or adolescents in their homes. This role focuses on the medical needs and treatment of patients according to their active plan of care, involving ongoing patient assessments, administration of prescribed medications and therapies, care coordination, family education, and preventative initiatives to ensure quality care. The agency has been serving the medically fragile community since 2000 and emphasizes values of Heart, Advocacy, Love, Outreach, and Speed.
Requirements
- Active RN or LPN/LVN license
- Provide care in a client home setting
- Ability to make a positive and lasting impression!

Responsibilities
- Focus on the medical needs and treatment of infants, children, and/or adolescents primarily in their home according to the active plan of care, as given by the physician and other appropriate personnel.
- Conduct on-going patient care and assessments.
- Administration of prescribed medication, treatments, and therapies.
- Coordination of care
- Educate family members on patient clinical care to enhance positive outcomes
- Preventative initiatives to protect quality of care for patient
