About The Position

Long Term Care Nurse Overview We are seeking a dedicated Long Term Care Nurse to join our healthcare team. The ideal candidate will provide comprehensive nursing care in a long-term care facility located within a hospital environment. This position involves performing assessments, administering treatments, and managing various patient care responsibilities.

Requirements

• Active Hawaii nursing license prior to the start of the assignment.
• Ability to arrive by the Thursday before the start date for an employee health appointment scheduled for Friday.

Responsibilities

• Conduct patient assessments and develop individualized care plans.
• Administer medications and monitor patients' responses to treatment.
• Communicate pertinent information to Medical Doctor (MD) or Nurse Practitioner (NP) as required.
• Assist with patient admissions and discharges.
• Support patients with activities of daily living (ADLs).
• Provide care for gastrostomy tubes and urinary catheters.
• Administer nebulizer treatments and manage wound vac therapy.
• Oversee CPAP care for patients as needed.
• Perform other assigned duties in accordance with patient care protocols.
