RN/LPN - The Grand of Dublin
About The Position
Optalis Health & Rehabilitation is seeking an RN or LPN for The Grand of Dublin, a facility located at Bridge Park. The role ensures residents receive appropriate personal care and are supported in attaining their optimum level of functioning, while helping the center comply with organizational and local, state, and federal regulations. The nurse coordinates resident care with the Administrator, Director of Nursing, and other disciplines; supports quality care and survey readiness; makes clinical decisions to promote resident outcomes; responds to call lights and resident needs; and communicates pertinent care information between shifts. The posting lists full-time and part-time opportunities and notes weekend and night shift differentials.
Requirements
- Possess a Licensed Practical Nurse license from the state in good standing.
- High school diploma or GED.
- CPR certification.
- Effective communication and documentation abilities.
- Organizational and time-management skills.
- Ability to lift 80–300 lbs with assistance or equipment.
- Frequent standing and walking, and ability to respond to emergencies.
- Leadership and accountability, integrity and professionalism, adaptability, and a resident-centered approach.
Nice To Haves
- Two years of experience in long-term care and management of resident care.
Responsibilities
- Coordinate resident care with the Administrator, Director of Nursing, and other disciplines to ensure quality care.
- Ensure care plans are implemented and updated based on resident needs.
- Make effective decisions to maximize clinical outcomes for residents.
- Answer call lights and communicate resident needs to the appropriate person when unable to meet them independently.
- Prioritize call-light requests and ensure they are addressed in a timely manner.
- Support survey readiness and quality assurance activities.
- Complete daily rounds with the incoming shift.
- Provide the oncoming personnel with an accurate, concise, and pertinent shift report.
- Coordinate the timely and appropriate dissemination of resident information.
- Help ensure the center complies with organizational and local, state, and federal regulations.
