About The Position

Licensed Practical Nurse (LPN) or Registered Nurse (RN) - Noble Man Care Services Location: Spring Hill, FL (Serving Hernando, Citrus, Levy, Sumter, Marion, and Lake counties) Employment Type: Full-time/Part-time Schedule: Mon-Fri 10 AM-6 PM, with 24/7 on-call rotation About Noble Man Care Services Join our innovative family-centered companion care and clinical home health hybrid agency. We provide premium clinical oversight with 24/7 APRN availability, serving clients with complex chronic conditions including diabetes, heart disease, hypertension, and post-operative care needs. Position Overview We're seeking a compassionate and skilled LPN or RN to join our growing team. You'll provide direct patient care in home settings while working under the supervision of our experienced APRN owner with 25+ years in healthcare.

Requirements

Current Florida LPN or RN license
CPR BLS certification
Home Health Aide certification preferred
Experience with chronic disease management (diabetes, hypertension, heart disease)
Proficiency with wound care and post-operative care
Reliable transportation and valid driver's license

Nice To Haves

Home health or community health experience
Experience with OASIS assessments
Familiarity with EHR systems (Alora, Charm Health, eClinical)
Bilingual capabilities (Spanish preferred)

Responsibilities

Vital sign monitoring, blood sugar testing, medication administration, injections, wound care, dressing changes
Ostomy/catheter care, chronic condition management, post-surgical care
Complete OASIS assessments, care plans, and status reports using Alora, Charm Health, and eClinical systems
Medication compliance, disease management, fall prevention
Participate in 30-minute urgent response protocol
Participate in QAPI meetings and continuous improvement initiatives
