About The Position

The Visiting Nurse Association of Florida (VNA) is a 50-year-old home health agency that is growing and looking to hire Registered Nurses (RN) in the Hobe Sound area. As an LPN or RN, you will work with referral sources from the healthcare community to implement services to meet the needs of individuals and families in their homes. You will be responsible for completing physical assessments and determining any patient home care needs, assisting in developing and implementing that plan of care for each patient while using VNA of Florida's case management model.

Requirements

• MUST have Trach/Vent and GTube/ Supra Pubic experience
• Minimum of 1-year Clinical Experience
• Availability Monday, Tuesday and Thursday (9 pm- 6 am)
• Some weekend Availability

Responsibilities

• Work with referral sources from the healthcare community to implement services to meet the needs of individuals and families in their homes
• Completing physical assessments and determine any patient home care needs
• Assist in developing and implementing that plan of care for each patient while using VNA of Florida's case management model
• Providing treatment and care to patients in their home
