About The Position

The primary purpose of this job position is to provide direct nursing care to residents and to supervise the day-to-day nursing activities performed by nursing assistants. This supervision must be in accordance with the Florida Nurse Practice Act, current federal, state, and local standards, guidelines, and regulations. The role involves administrative, charting, drug administration, personnel, nursing care, staff development, safety, sanitation, equipment, supply, care plan, assessment, and resident rights functions. The position requires adherence to facility and departmental policies and procedures, and other duties as assigned.

Requirements

  • Graduate of an accredited school of nursing.
  • Must possess and maintain an “Eligible” ACHA Background Screening status.
  • Valid and current license as a RN or LPN in the State of Florida.
  • Excellent communication and interpersonal skills.
  • Must be able to read, write, speak, and understand the English language.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
  • Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
  • Must possess leadership ability and the willingness to work harmoniously with and supervise other personnel.
  • Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, etc. that are necessary for providing quality care.
  • Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.
  • Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.
  • Must be able to relate information concerning a resident's condition.
  • Must not pose a direct threat to the health or safety of other individuals in the workplace.
  • Must be able to move intermittently throughout the work day.
  • Must be able to speak, read and write the English language in an understandable manner.
  • Must be able to cope with the mental and emotional stress of the position.
  • Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately.
  • Must function independently, have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.
  • Must meet the general health requirements set forth by the policies of this facility which include a medical and physical examination.
  • Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people within the facility.
  • Must be able to twist and turn all extremities and at the waist.
  • Must be able to push, pull, move, and/or lift a minimum of 35 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 450 feet.
  • Hours and days may be varied at times as necessary to maintain good nursing care.
  • May be necessary to assist in the evacuation of residents during emergency situations.

Nice To Haves

  • One (1) year of experience as a nurse in a health care facility preferred.

Responsibilities

  • Direct the day-to-day functions of the nursing assistants in accordance with current rules, regulations, and guidelines.
  • Ensure that all nursing personnel assigned comply with facility policies and procedures and the Nursing Service Procedures Manual.
  • Participate and cooperate in facility surveys (inspections) by government agencies.
  • Admit, transfer, and discharge residents in accordance with facility protocols.
  • Make written and oral reports/recommendations concerning shift activities.
  • Cooperate with other resident services/departments to ensure the resident's total regimen of care is maintained.
  • Periodically review and assist in updating the resident's written discharge plan.
  • Complete accident/incident reports and submit them to the Director of Nursing or designee.
  • Complete and file required recordkeeping forms/charts upon resident's admission, transfer, and/or discharge.
  • Receive telephone orders from physicians and record in the EMR system.
  • Transcribe physician's orders to resident’s EMR, card-ex, medication cards, and treatment care plans.
  • Chart nurses' notes in an informative and descriptive manner reflecting care provided and resident's response.
  • Chart all reports of accidents/incidents involving residents.
  • Record new/changed diet orders and forward information to the Dietary Department.
  • Report discrepancies concerning physician's orders, diet changes, charting errors, etc., to the Nurse Supervisor/Unit Manager.
  • Fill out and complete transfer forms in accordance with established procedures.
  • Ensure appropriate documentation concerning unauthorized discharges is entered in the resident's medical record.
  • Perform routine charting duties as required.
  • Prepare and administer medications as ordered by the physician.
  • Verify resident identity before administering medication/treatment.
  • Ensure prescribed medication for one resident is not administered to another.
  • Ensure an adequate supply of floor stock medications, supplies, and equipment is on hand.
  • Order prescribed medications, supplies, and equipment as necessary.
  • Ensure that narcotics are handled and records are accurate in accordance with laws and facility policies.
  • Notify the Nurse Supervisor/Unit Manager of all drug and narcotic discrepancies.
  • Review medication cards for completeness, accuracy, and adherence to stop order policies.
  • Notify the attending physician of automatic stop orders prior to the last dosage being administered.
  • Dispose of drugs and narcotics as required.
  • Participate in employee performance evaluations and determine staffing requirements.
  • Inform the Nurse Supervisor of staffing needs when assigned personnel fail to report to work.
  • Report absentee call-ins to the Nurse Supervisor/Unit Manager.
  • Develop work assignments and/or assist in completing and performing such assignments.
  • Monitor Nursing Assistants to ensure care and services are provided according to the resident’s plan of care.
  • Report problem areas to the Nurse Supervisor/Unit Manager.
  • Develop and maintain good working rapport with inter-department personnel.
  • Create and maintain an atmosphere of warmth, personal interest, and positive emphasis.
  • Review complaints and grievances made or filed by assigned personnel and make appropriate reports.
  • Receive/give the nursing report upon reporting in and ending shift duty hours.
  • Report occupational exposures to blood, body fluid, infectious materials, and hazardous chemicals.
  • Inform nursing service personnel of new admissions and ensure rooms are ready.
  • Greet newly admitted residents and escort them to their rooms.
  • For LPNs, observe and document patient status and report concerns to the RN.
  • For RNs, assess and document patient status.
  • Participate in the orientation of new residents/family members.
  • Make rounds with physicians as necessary.
  • Requisition and arrange for diagnostic and therapeutic services as ordered by the physician.
  • Consult with resident's physicians regarding resident's care, treatment, and rehabilitation.
  • Review resident's chart for specific treatments, medication orders, diets, etc.
  • Implement and maintain established nursing objectives and standards.
  • Make periodic checks to ensure prescribed treatments are administered and evaluate resident's status.
  • Ensure direct nursing care is provided in accordance with the resident’s plan of care and wishes.
  • Notify the resident's attending physician of accidents, incidents, or changes in condition.
  • Notify the resident's attending physician and next-of-kin when there is a change in the resident's condition.
  • Carry out restorative and rehabilitative programs.
  • Administer professional services such as catheterization, tube feedings, suction, dressing changes, etc.
  • Use restraints only when prescribed and in accordance with policies.
  • Obtain specimens for lab tests as ordered.
  • Take and record TPRs, blood pressures, etc.
  • Monitor seriously ill residents as necessary.
  • Check food brought into the facility by family/visitors.
  • Promptly respond to call bells and requests for assistance.
  • Ensure residents who are unable to call for help are checked frequently.
  • Meet with residents and/or family members as necessary.
  • Admit, transfer, and discharge residents as necessary.
  • Assist in arranging transportation of discharged residents.
  • Ensure discharged residents are escorted to the pick-up area.
  • Inform family members of the death of the resident.
  • Call funeral homes when requested by the family.
  • Ensure established post-mortem procedures are followed.
  • Attend and participate in all facility in-service trainings/programs.
  • Attend and participate in annual OSHA and CDC in-service training programs.
  • Attend and participate in advance directive in-service training.
  • Attend and participate in continuing education programs.
  • Monitor personnel to ensure they are following established safety regulations.
  • Ensure departmental policies and procedures are followed by self and assigned personnel.
  • Ensure assigned work areas are maintained in a safe, clean, and sanitary manner.
  • Ensure unit's resident rooms, treatment areas, etc., are maintained in a clean, safe, and sanitary manner.
  • Ensure safety policies and procedures are followed.
  • Ensure hand-washing procedures are followed.
  • Ensure participation in fire safety and disaster preparedness drills.
  • Ensure infection control procedures are followed when isolation precautions are necessary.
  • Ensure safety equipment and supplies are used when moving or lifting a resident.
  • Report missing/illegible labels and MSDSs to the safety officer.
  • Recommend equipment and supply needs of the unit/shift.
  • Ensure an adequate stock level of medications, medical supplies, and equipment is maintained.
  • Ensure only trained and authorized personnel operate unit/shift's equipment.
  • Ensure nursing service equipment is operated in a safe manner.
  • Review care plans daily to ensure appropriate care is rendered.
  • Inform the Nurse Supervisor/Unit Manager of any changes needed on the care plan.
  • Ensure nurses' notes reflect that the care plan is being followed.
  • Review resident care plans for appropriate goals, problems, approaches, and revisions.
  • Ensure assigned LPNs and CNAs are aware of resident care plans and carry them out.
  • Assist the Resident Assessment/Care Plan Coordinator with MDS, RAPs, and Triggers.
  • Maintain the confidentiality of all resident care information.
  • Monitor nursing care to ensure residents are treated fairly, with kindness, dignity, and respect.
  • Ensure all nursing care is provided in privacy and personnel knock before entering rooms.
  • Ensure personnel are knowledgeable of resident rights and responsibilities.
  • Review resident complaints or grievances and report actions taken to resolve them.
  • Maintain a written record of resident complaints and/or grievances.
  • Report all allegations of resident abuse and/or misappropriation of resident property.
  • Honor resident's refusal of treatment requests in accordance with policies.
  • Adhere to facility and departmental policies and procedures.
