About The Position
Horizon Health is seeking a full-time Registered Nurse, Licensed Practical Nurse, or Certified Medical Assistant to provide nursing care to patients in an outpatient clinic setting in the Southeastern Region, which includes Yankton, Alcester, and Elk Point. The organization is looking for candidates passionate about delivering high-quality, patient-centered care. The role coordinates care throughout the patient visit, provides patient education and care coordination, and performs nursing skills within the scope of the employee’s license or certification. Requirements
- Registered Nurses and Licensed Practical Nurses must have a valid South Dakota nursing license.
- Medical Assistants must be certified through an accredited program.
Responsibilities
- Coordinate care throughout the patient visit, including rooming patients, taking vital signs, collecting history, updating medications and clinical information in the EHR, preparing patients for provider visits, scheduling follow-up care as needed, and cleaning and stocking rooms.
- Provide patient education on overall health management.
- Initiate referral management and coordinate care for preventive and specialty visits.
- Develop care plans, including self-management plans and goals.
- Initiate and perform nursing skills within the scope of the employee’s license, including provider-ordered injections, vaccinations, labs, X-rays, and other tests.
