About The Position

Horizon Health is seeking a full-time Registered Nurse, Licensed Practical Nurse, or Certified Medical Assistant to provide nursing care to patients in an outpatient clinic setting. The organization seeks candidates who are passionate about high-quality, patient-centered care. The work environment is collaborative and team-oriented, focused on individual patient needs and supporting community health and wellness. The Southeastern Region includes Yankton, Alcester, and Elk Point. The role coordinates care throughout the patient visit, provides patient education and care coordination, and performs nursing skills within the scope of the applicable license. On-the-job training is provided for labs and X-rays.

Requirements

A valid South Dakota nursing license is required for nursing applicants.
Medical Assistants must be certified through an accredited program.

Responsibilities

Coordinate patient care throughout the visit, including rooming patients, taking vital signs, collecting an adequate history, updating medications and clinical information in the EHR, preparing patients for provider visits, scheduling follow-up care as needed, and cleaning and stocking rooms.
Provide patient education on overall health management.
Initiate referral management and coordinate care for preventive and specialty visits.
Develop care plans, including self-management plans and goal initiation.
Initiate and perform nursing skills within the scope of the applicable license, including provider-ordered injections, vaccinations, labs, X-rays, and other tests.
