About The Position

The RN Liaison is responsible for ensuring that patients referred to Evergreen Home Care Services (EHCS) have a smooth transition to Home Care services in the care setting of their choice. This role involves educating the community, physicians, and other healthcare providers about Hospice, Palliative Care, Home Health, and Behavioral Health services. The position is responsible for the growth of the referral base and consistent improvement in community reputation as a leader of Home Care services. The RN Liaison assesses the appropriateness for Home Care Services for patients in hospitals, skilled nursing facilities, adult family homes, and assisted living facilities. They interact and engage daily with hospital staff, skilled nursing facility staff, physician office staff, and the Home Care administrative and clinical staff. This role initiates and maintains collaborative relationships and clear communication between EHCS management and referral sources. The RN Liaison participates in customer service measurements and functions, collecting, analyzing, and sharing information with EHCS management. They are responsible for maintaining and growing existing and new relationships with referral sources across the care continuum and throughout the EHCS territory. Additionally, the RN Liaison acts as a resource to provide comprehensive solutions to the community and other stakeholders. All candidates must live in WA state and be able to come to our Kirkland hospital main campus for training/orientation.

Requirements

Graduate of a nursing program (Diploma or Associate Degree in Nursing (ADN) required)
Current BLS Healthcare Provider certification by date of hire.
2 years of Home Health, Hospice or other Home Care experience.
Must have prior marketing, sales and/or community outreach experience.
Current Washington State Driver’s license and proof of insurability, if the position requires regular driving.
Must have working knowledge of Medicare COP, JC Standards
Must be familiar with Third party insurance and Medicare HMO’s.
Must live in WA state and be able to come to our Kirkland hospital main campus for training/orientation.
