This role coordinates incoming office referrals and verifies insurance coverage. It also coordinates incoming calls from patients, family, physicians, and disciplines. The position ensures that all personnel are informed of patient's hospitalization and follow-up with patients admitted to other hospitals. The RN Intake Coordinator coordinates small programs with limited budget/impact. This role applies substantial knowledge and experience to perform a wide range of advanced activities and/or determines how to use resources to meet schedules and goals; serves as working supervisor for team or work group.

Requirements

  • High School Education/GED or equivalent: Required

Nice To Haves

  • Associate’s/Technical Degree or equivalent combination of education/related experience: Preferred
  • Three years' relevant healthcare experience: Preferred
  • Registered Nurse (RN) or Licensed Vocational Nurse (LVN) licensure in state of practice: Preferred

Responsibilities

  • Coordinates incoming office referrals and verifies insurance coverage.
  • Coordinates incoming calls from patients, family, physicians, and disciplines.
  • Ensures that all personnel are informed of patient's hospitalization and follow-up with patients admitted to other hospitals.
  • Coordinates small program(s) with limited budget/impact.
  • Applies substantial knowledge and experience to perform a wide range of advanced activities and/or determines how to use resources to meet schedules and goals; serves as working supervisor for team or work group.
  • Handles and coordinates incoming calls related to patients, physicians, referrals and agency services regarding physician orders, patient questions, and patient referrals.
  • Facilitates telephone communications between, and demonstrates effective communication skills, with nursing, therapy, aide, social services, physicians, etc. regarding changes in patient/staff schedule, test results, etc.
  • Assists with staffing/scheduling activities, soliciting input from managers.
  • Provides follow-up for hospitalized patients, ensuring subsequent agency referral.
  • Performs other job-related duties as assigned.
