LPN Intake Coordinator - Home Health
About The Position
The LPN Intake Coordinator plays a key role in the coordination of patient care by managing the intake process for home health or hospice services. Working onsite in the office, this position involves reviewing referral information, obtaining necessary clinical documentation, and ensuring all intake requirements are met in accordance with physician orders and the patient's plan of care. The LPN Intake Coordinator collaborates closely with physicians, clinical staff, and referral sources under the supervision of the Vice President of Home Health Operations to ensure a smooth and timely start of care for each patient.
Requirements
- LPN license
- Experience in home health or hospice intake
- Familiarity with physician orders and patient plans of care
- Ability to collaborate with physicians, clinical staff, and referral sources
