The RN - Immediate Care will develop a plan of care that identifies interventions to achieve expected outcomes, and evaluates patient’s progress toward attainment of outcomes, systematically revising diagnosis, interventions and expected outcomes as needed. This role provides patient assessment and collection of health data involving the patient, family, and other health care providers. The RN will communicate the plan of care to the patient and family regularly, respecting the patient’s wishes through acknowledgment of patient/family perceptions of health and illness and mutual goal setting for optimal outcomes. The RN will also communicate patient data to the physician and interdisciplinary team as indicated to achieve patient care needs and document patient health data, interventions, and communications. Individualized patient and family teaching will be provided based on a thorough assessment to meet their specific learning needs, assessing the patient’s cultural background, learning style, learning barriers, and modifying the teaching plan accordingly. Teaching on disease management strategies, emergency care, and drug information will be provided when appropriate. The RN will identify community resources to support patient self-management of disease. This role will interact with and contribute to the professional development of peers, students, and other health care providers as colleagues, sharing knowledge, providing feedback, and contributing to an environment supportive of clinical education. Tasks may be assigned or delegated as needed. The RN will nurture a compassionate environment by providing psychological support and provide assistance to technical staff with injection of contrast, venipuncture, and administration of drugs, O2 therapy, and IV management. Participation in performance improvement activities and nursing research is expected. The RN will keep abreast of research and practice trends in wound/ostomy/skin care through participation in professional networking, ongoing education, and self-directed learning activities, and support staff in using research and providing evidence-based care. If skilled, the RN will conduct daily rounds of wound/ostomy/skin care patients, monitor the effectiveness of wound/ostomy treatment plans and other aspects of wound/ostomy/skin care, and comply with HIPAA regulations. Assistance in the transporting of acutely ill patients may be needed.
