About The Position

The RN - Immediate Care position is a full-time role requiring 38 hours per week, with operating hours from Monday to Friday, 8 AM to 8 PM, and weekends/holidays from 8 AM to 4 PM. This role involves developing and evaluating patient care plans, providing patient assessments, and communicating care plans to patients and families. The nurse will document patient health data, provide individualized patient and family teaching, and identify community resources to support patient self-management. Additionally, the RN will contribute to the professional development of peers and students, delegate tasks, provide psychological support, and assist technical staff with various medical procedures. The role also includes participation in performance improvement activities, nursing research, and staying updated on wound/ostomy/skin care trends. If skilled, the RN will conduct daily rounds for wound/ostomy/skin care patients and monitor treatment effectiveness. Compliance with HIPAA regulations and assisting in transporting acutely ill patients are also key responsibilities.

Requirements

  • Bachelors Degree Nursing Required
  • Registered Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR) Active Illinois State Nursing License Required
  • Advanced Cardiovascular Life Support (ACLS) - American Heart Association (AHA) Required
  • Basic Life Support (BLS) - American Heart Association Required

Nice To Haves

  • 1 Year ICU or ER experience, preferred
  • 1 Year radiology nursing experience, preferred
  • 1 Year Wound/ostomy experience, preferred
  • Certified Wound Ostomy Nurse (CWON) - Wound, Ostomy, Continence Nursing Certification Board (WOCNCB) Preferred
  • Certified Wound Care Nurse (CWCN) - Wound, Ostomy, Continence Nursing Certification Board (WOCNCB) Preferred
  • Successful completion of Wound Ostomy Associate class, preferred

Responsibilities

  • Develops a plan of care that identifies interventions to achieve expected outcomes.
  • Evaluates patient’s progress toward attainment of outcomes, systematically revises diagnosis, interventions and expected outcomes as needed.
  • Provides patient assessment and collection of health data involving patient, family and other health care providers.
  • Communicates plan of care to patient and family regularly respecting the patient’s wishes through acknowledgment of patient/family perceptions of health and illness and mutual goal setting for optimal outcomes.
  • Communicates patient data to physician and interdisciplinary team as indicated to achieve patient care needs.
  • Documents patient health data, interventions and communications.
  • Individualizes patient and family teaching based on a thorough assessment to meet their specific learning needs.
  • Assesses patient’s cultural background, learning style, learning barriers, and modifies teaching plan accordingly.
  • Provides teaching on disease management strategies, emergency care and drug information when appropriate.
  • Identifies community resources to support patient self-management of disease.
  • Interacts with and contributes to professional development of peers, students and other health care providers as colleagues.
  • Shares knowledge, provides feedback and contributes to an environment supportive of clinical education.
  • Assigns or delegates tasks as needed.
  • Nurture a compassionate environment by providing psychological support
  • Provide assistance to technical staff with injection of contrast, venipuncture, and administration of drugs, 02 therapy, and IV management.
  • Participates in performance improvement activities and nursing research.
  • Keeps abreast of research and practice trends in wound/ostomy/skin care through participation in professional networking, ongoing education, and self-directed learning activities.
  • Supports staff in using research and providing evidence-based care
  • If skilled, conducts daily rounds of wound/ostomy/skin care patients.
  • Monitors effectiveness of wound/ostomy treatment plans and other aspects of wound/ostomy/skin care
  • Complies with HIPAA regulations.
  • Assist in the transporting of acutely ill patients as needed.
