About The Position

The RN Hospice Specialty Registered Nurse ensures quality and safe delivery of hospice nursing services to patients in the home or assisted living setting. This role involves developing, implementing, and evaluating the plan of care, making necessary revisions, and coordinating care with other health professionals. The nurse will identify interdisciplinary care needs and recommend additional home health care services. Responsibilities include supervising Licensed Practical Nurses and Home Health Aides, conducting supervisory visits, and providing education, supervision, and counseling to clients and their families regarding home nursing procedures and other care needs. The nurse will also ensure client and family involvement in the plan of care development. Masonicare utilizes a state-of-the-art electronic medical records system, requiring timely and accurate computer documentation of patient/job information.

Requirements

Current RN License required.
NEW GRADS WILL NOT BE CONSIDERED.

Nice To Haves

Hospice AND/OR Home Care experience IS PREFERRED.

Responsibilities

Ensures quality and safe delivery of hospice nursing services to patients in the home or assisted living setting.
Develops, implements, and evaluates the plan of care and makes necessary revisions.
Provides coordination of the plan of care, maintaining continuity of client care with other health professionals.
Identifies interdisciplinary care needs and makes recommendations for additional home health care services.
Supervises the client care rendered by Licensed Practical Nurses and Home Health Aides in the provision of delegated duties and conducts supervisory visits per regulatory guidelines.
Provides education, supervision, and counseling to the client and their family regarding home nursing procedures and other care needs.
Assures that the client and their family are involved in the development of the plan of care whenever possible.
Demonstrates timely and accurate computer documentation of patient/job information.
