This role involves providing comprehensive hospice care to patients in their homes. The RN Case Manager will explain patient rights and advanced directives, coordinate care, and assess patients' physical and psychosocial needs. Responsibilities include developing and implementing care plans, instructing and supervising hospice aides and LPNs, and ensuring patients have necessary medical equipment and supplies. The nurse will also administer medications, provide patient and family education, and offer support during difficult times. Chart documentation must be completed within 48 hours, and the nurse will manage a caseload to meet agency standards. Collaboration with the Nursing Supervisor and other health team members is crucial for seamless patient care. The role also includes participation in an on-call rotation and adherence to infection control and confidentiality policies. The nurse must maintain a professional appearance and attend required in-service programs and meetings.
