About The Position

The Hospice Intake RN is responsible for coordinating all aspects of the referral-to-admission process to ensure timely, patient-centered, and cost-effective hospice services can be delivered efficiently. In collaboration with the Care Access Team, Admission RNs, referral partners, providers, patients, and families, the Hospice Intake RN facilitates the transition of patients into hospice care. Responsibilities include obtaining and reviewing clinical information, coordinating provider orders, completing eligibility reviews, scheduling admissions, entering and maintaining accurate clinical information within the electronic medical record, coordinating medication review and pharmacy processes, including medication order entry as well as serving as a clinical resource throughout the admission process. The Hospice Intake RN prioritizes referrals based on patient needs and collaborates with interdisciplinary team members to ensure responsiveness, regulatory compliance, and exceptional service to patients and families.

  • High School Diploma/GED
  • RN Degree, BSN preferred.
  • Minimum of 2 years clinical experience
  • Minimum of 1 year hospice experience
  • Demonstrated superior customer service skills
  • Demonstrated skill/expertise in pain and symptom management
  • Demonstrated ability to effectively collaborate and facilitate the work of the interdisciplinary group
  • Demonstrated ability to execute an effective, supportive, and sensitive response to needs of patients and their families and to staff members
  • Valid North Carolina RN license
  • Valid driver’s license and proof of automobile insurance
  • Prolonged sitting, in excess of four hours per day
  • Ability to drive and/or transport self to and from work locations
  • Hand dexterity to include gripping, carrying, pushing & pulling
  • Fine finger movements to include writing & typing, frequently
  • Flexion/Extension of arms & wrists
  • Additional gross motor movements including bending, reaching, twisting, turning, crouching, squatting, & kneeling
  • Near & far visual acuity
  • Ability to hear, understand, & distinguish speech and other sounds

Responsibilities

  • Practice within the scope of licenses, certification, or registration, where applicable, and as required by law and regulation
  • Obtain all necessary and required clinical information for all incoming referrals in coordination with the Care Access Team
  • Obtain and coordinate verbal orders from referral partners as needed
  • Contact families and schedule hospice admissions
  • Complete hospice eligibility reviews and medical criteria summaries, present findings to hospice physicians or advanced practice providers, and obtain admission approval as appropriate.
  • Coordinate provider face to face visits with the provider scheduler as required
  • Triage all informational calls and coordinate scheduled visits with applicable clinicians
  • Daily collaboration with Team Leaders and families to ensure timely start of care for all pending referrals
  • Perform other but similar duties, such as providing emergent on call patient needs coverage
  • Complete pre-admission screening to determine the correct FS service line
  • Coordinate and obtain consents as needed
  • Assist with Triage or other Care Access Tasks during lower referral volumes
  • Review referral documentation and enter medication profiles into the EMR to support hospice eligibility review, admission preparation, and continuity of care.
  • Collaborate with pharmacy providers, physicians, and Admission RNs to ensure timely medication review, coverage determination, and medication availability prior to admission whenever possible.
  • Perform clinical review of referrals, including diagnosis, medication profile, and supporting documentation, to facilitate efficient admission planning.
  • Review referrals for hospice appropriateness and identify clinical barriers, medication concerns, equipment needs, or other issues that may impact the admission process.
  • Practice safety and infection control methods
  • Participate in the coordination of the patient’s hospice care in accordance with professional standards of practice
  • Share in providing 24-hour, 7 day per week coverage to patient’s/families by participating in on-call rotation when needed, weekend and holiday coverage per department and agency requirements
  • Attend staff meetings and mandatory in-services as required
  • Participate in ongoing education and assumes the responsibility for updating professional skills and maintaining of licensure
