About The Position

Join The Samaritan Difference! This role is responsible for the coordination and implementation of the referral process and the coordination and initiation of the admissions process. You will work collaboratively with Nurse Managers to ensure optimal hospice care, and collaborate with the Manager on complex referrals. This position also involves assigning pending referrals and portals to staff, facilitating daily huddles to assess referral activity, and ensuring mission values are integrated into daily work. You will utilize excellent interpersonal skills for customer satisfaction, function as a resource, mentor, and coach for clinical staff, and manage resources for quality care within cost-effective parameters and regulatory standards. Additionally, you will manage phone calls and communications with the healthcare team and assist the manager with the monthly schedule for referral center staff. There is a weekend obligation of one weekend per month and one holiday per year.

Requirements

  • Valid RN license in NJ
  • 5 years of medical/surgical experience required

Nice To Haves

  • BSN preferred
  • 1 year of hospice/palliative care or three years of oncology experience preferred
  • OCN, CRNH, or Board Certification in Nursing Professional Development preferred

Responsibilities

  • Functions as resource person, mentor and coach for clinical staff
  • Manages resources to provide quality delivery of care within cost-effective parameters and regulatory standards
  • Manages phone calls and communications with other members of the health care team
  • Assists manager with monthly schedule for referral center staff
  • Weekend obligation of one (1) weekend per month as well as one (1) holiday per year
