About The Position

Samaritan is a leading not-for-profit, locally based provider of home-based primary care, palliative care, hospice care, grief support, education, social connections and advocacy. Proudly serving the South Jersey community since 1980. The RN - Hospice Intake Lead is responsible for the coordination and implementation of the referral process and the coordination and initiation of the admissions process. This role works collaboratively with Nurse Managers to ensure optimal hospice care and collaborates with the Manager on complex referrals. The position also involves assigning pending referrals and portals to referral staff, facilitating daily huddles to assess referral activity, and ensuring mission values are translated into daily work. The RN - Hospice Intake Lead utilizes excellent interpersonal skills for customer satisfaction, functions as a resource, mentor, and coach for clinical staff, and manages resources for quality care within cost-effective parameters and regulatory standards. They also manage phone calls and communications with the healthcare team and assist the manager with the monthly schedule for referral center staff.

Requirements

  • Valid RN license in NJ
  • 5 years of medical/surgical experience required

Nice To Haves

  • BSN preferred
  • 1 year of hospice/palliative care or three years of oncology experience preferred
  • OCN, CRNH, or Board Certification in Nursing Professional Development preferred

Responsibilities

  • Responsible for coordination and implementation of the referral process and the coordination and initiation of the admissions process

  • Assists manager with monthly schedule for referral center staff
