About The Position

CareFor is seeking a highly organized and compassionate Hospice Intake Coordinator to support our hospice team from the initial referral through admission. This position combines intake responsibilities with general office administration, so it is well suited for someone who enjoys staying busy, keeping details on track, and helping things run smoothly behind the scenes. You will work closely with our community liaisons, referral sources, patients, families, and clinical team. You will also help keep our office organized and adequately supplied. This is a full-time, in-office position.

Requirements

  • Previous experience in healthcare, home care, hospice, or a medical office environment.
  • Strong organizational skills and exceptional attention to detail.
  • The ability to prioritize competing responsibilities and maintain a reliable system for tracking follow-ups.
  • A professional and compassionate phone presence, particularly when communicating with patients and families during a difficult time.
  • Comfort working with electronic documentation and communication tools such as DocuSign, Adobe, Microsoft Teams, and EMR systems.
  • The confidence to communicate with referral sources and internal team members, ask for missing information, and follow up when something is needed to move a referral forward.
  • A dependable, self-directed approach to work and a willingness to pitch in wherever needed.

Nice To Haves

  • Hospice experience is helpful, but we are open to training someone with the right skills and attitude.
  • Familiarity with HIPAA and handling confidential medical information is a plus.

Responsibilities

  • Serve as a primary point of contact for community liaisons and incoming hospice referrals, responding to calls and requests promptly and professionally.
  • Receive and organize new referrals and complete the benefits verification process.
  • Manage referrals from initial intake through admission, tracking each step and following up to keep the process moving.
  • Send and track release-of-information requests and consent forms to patients and family members through DocuSign.
  • Gather required medical information and records from referral sources, organize the documentation, and enter information into the EMR.
  • Coordinate with patients and their representatives, the Director of Clinical Services, and members of the Interdisciplinary Team to support a smooth transition to hospice care.
  • Maintain accurate and confidential records in accordance with HIPAA, Texas Health and Human Services requirements, and CMS hospice regulations.
  • Participate in routine Interdisciplinary Team meetings.
  • Answer and route calls on the hospice phone line.
  • Order and maintain appropriate inventory of medical, hygiene, and incontinence supplies.
  • Manage kitchen and office supply orders for the team.
  • Receive and distribute packages and deliveries.
  • Provide administrative support to help keep the office organized and running efficiently.
